GentleCheck is a patient satisfaction platform that works with licensed home healthcare agencies to check in on patients during and after their care. We send automated text messages (SMS) and voice calls on behalf of your healthcare agency to ask how your care is going and to collect your feedback.
Your home healthcare agency has partnered with GentleCheck to reach out to you about your care experience. Here is how the process works:
Step 1 — You receive an opt-in message.
GentleCheck will send you a single text message introducing ourselves and identifying the healthcare agency that provides your care. This message will ask if you would like to participate in brief satisfaction check-ins via text message. The opt-in message does not contain any health information.
Step 2 — You choose whether to participate.
To consent to receiving satisfaction survey messages, reply YES to the opt-in message. Participation is completely voluntary. If you do not reply or do not wish to participate, you will not receive further messages.
Step 3 — You receive check-in messages.
Once you opt in, GentleCheck will send you short survey questions about your care experience at various points during and after your care episode. You respond by replying directly to the text messages. Your responses are shared with your healthcare agency to help them monitor and improve the quality of your care.
Message frequency varies. The number of messages you receive depends on the check-in schedule set by your healthcare agency. You may receive messages at the start of your care, periodically during your care, and after your care ends. You will typically receive no more than a few messages per week during active check-in periods.
You can stop receiving text messages from GentleCheck at any time. To opt out, reply STOP to any GentleCheck text message. You will receive a single confirmation message acknowledging your opt-out, and you will not receive any further messages from GentleCheck unless you choose to opt back in.

Standard message and data rates may apply depending on your mobile carrier and plan. GentleCheck does not charge you for receiving or sending text messages through our platform. Please check with your carrier if you have questions about your messaging plan.
The initial opt-in message you receive from GentleCheck does not contain Protected Health Information (PHI). It will identify GentleCheck and the name of your healthcare agency but will not include details about your health, diagnosis, treatment, or care.
